Key benefits: Lower out-of-pocket costs such as deductibles, co-pays and co-insurance; flexibility to see any provider in Anthem’s nationwide network

Considerations: Higher monthly premiums in exchange for lower costs when you receive care and greater network flexibility

UK’s PPO plan offers greater flexibility to choose providers from UK HealthCare or Anthem’s nationwide network. UK HealthCare facilities and providers offer lower costs when you’re on the PPO plan.

The PPO plan also offers low out-of-pocket costs, including:

  • Deductible (the amount you must pay before insurance begins covering a share of costs)
  • Co-pays (a flat fee you pay when you receive a health care service)
  • Co-insurance (a percentage of the cost you pay when you receive a health care service)

2025-26 deductibles and out-of-pocket maximums

Currently, King's Daughters Medical Center in Ashland and St. Claire Regional Medical Center in Morehead are not part of the UK HealthCare network.

In rare circumstances – if you spend more than 120 days out of the country, for example – you may need to choose our Indemnity plan.

* Regular part-time and temporary employees (less than 0.75 FTE or work less than an average of 30 hours per week in a 12-month measurement period), who are not eligible for the UK credit toward the costs of coverage, pay this rate.

If you and your spouse both work at UK and are eligible for UK's credit toward health insurance, and you have dependents to cover on your health insurance plan, you may enroll in Combined Credit coverage at the employee + family level. The monthly rate on the PPO plan is $336.

Your first 30 days

Welcome to UK! You can sign up for benefits depending on your FTE. Enroll in your benefits quickly in myUK.

Qualifying events

If you experience a major life change outside of open enrollment you can also make changes to your insurance. This is known as a qualifying event. You'll need to fill out and submit a form within 30 days.

Benefits Open Enrollment

Our Benefits Open Enrollment happens every year so you can make changes. Changes can be made in myUK, or with paper forms.

Please use the Retiree Benefits Enrollment form and return to Bosworth Hall.

Change to our pharmacy network

Kroger has announced it will withdraw from the national network of Express Scripts, which manages our pharmacy benefit through our health insurance plans. This was a national decision by Kroger that takes effect January 1.  You can view the full list of Express Scripts in-network pharmacies here.

FSA debit cards now usable at all UK Retail Pharmacies

We have great news for employees with a health care flexible spending account or health savings account! You can now use the debit card that comes with your FSA or HSA at all UK Retail Pharmacies and UK Retail Pharmacy Mail Order. For quick reference, to request a prescription refill and for UKHC Pharmacy locations go to UK Retail Pharmacies. You can also reach any of the retail pharmacy locations by calling 833-333-UKRx (8579).
