Family Medical Leave Process for Supervisors
Supervisor becomes aware an employee may have an FML qualifying event. This need could be due to one of the following:
- Pregnancy
- Placement of a child with employee through adoption/foster care
- Misses more than three consecutive days of work due to a health condition
- Absent on an intermittent basis due to a health condition
- Overnight hospitalization
- Placement in hospice care
- Placement in a residential care facility
- Missing work for multiple appointments related to a health condition
- Is needed to care for a family member (spouse, parent, child, sponsored adult dependent, sponsored child dependent) with one of the above listed scenarios
Present the employee with the FML memo and application, notifying them of their rights to FML and giving them 15 calendar days to apply for FML. Make sure the employee signs the memo and a copy is sent to the FML office via email (LeaveAdminFMLA@uky.edu) or fax (859-257-2010 or 859-257-1679). The memo can be found here.
- The employee should never return the completed forms back to you. They need to be sent directly to the FML office.
- Once the employee returns back to work, their return back to work certificate must be sent to the FMLoffice.
Incomplete forms will not be processed.
