This form should not be used to request a remote or hybrid schedule. If you are interested in a remote or hybrid schedule, please talk with your supervisor. Plans for remote and hybrid work will be approved at the level of a direct report to the President.

  • Adjusted hours
  • Compressed work week
  • Reduced hours
    • Change a full-time position to part-time
    • Temporarily take unpaid leave
    • Temporarily reduce hours
    • Temporarily reduce hours after returning from extended leave, such as family medical leave
    • Phase into retirement by reducing hours. Please follow this process outlined here.

 

Dates & Information 

Adjusted hours

This flexibility exists only outside of core hours of business, as defined by each supervisor. Core hours may be mid-day from 10 a.m. to 2 p.m. when meetings take place, for example.

Compressed work week

A standard workweek can be compressed into fewer than five days. In some positions, working four 10-hour days or three 13-hour days is possible.

Reduced hours

Change a full-time position to part-time

Temporarily take seasonal unpaid leave

Temporarily reduce hours seasonally

Temporarily reduce hours after returning from extended leave, such as family medical leave

Phase into retirement by reducing hours

It's best to discuss your flex work request with your supervisor before submitting this form. Once you submit it, your supervisor will receive an email copy. He or she will reply to the email with "APPROVED" in the subject line and send a copy to you, as well as UK Human Resources. If you have not received a response from your supervisor within two days, you may want to ask them about your pending request.
