To release salary information for verifications of employment (VOE), use this form. The information cannot be provided until this form (or that of the requesting organization) has been signed and submitted. Note that businesses and non-government agencies will also be required to pre-pay a $30 fee, and a VOE cannot be completed until confirmation that the fee has been paid.
