With any qualifying event, including birth or adoption, you have 30 days from the date of the event to submit a new enrollment form, along with the required documentation.
To upload your documentation:
1. Log in to myUK to update your personal information.
2. To access the dependent verification upload feature, click on the "Benefits and Payment" menu item.
3. Locate the menu item named "Dependent Verification." Click the link underneath named "Upload Document Verification Here."
4. You will then upload documentation to the portal. Upload the documents necessary to verify any spouse, sponsored dependent, child or ward on your health, dental or vision insurance plans.
