UK HealthCare PE Guidelines
June 2008 for Budget 2009
Performance Evaluations are required for every employee listed on the Healthcare roster who meet the qualifications established. Forms and instructions can be found on the University eforms web site www.uky.edu/eforms.
The Healthcare Performance Evaluation process will be required to be completed with the following guidelines:
- Employee Review will be based on the timeframe of July 1, 2007 through June 30, 2008.
- Employees hired during the July 1, 2007 through June 30, 2008 timeframe:
- Employee new hire/transfer orientation must be completed and employee must have a performance score by June 30, 2008. (A score of 3.0 will be considered as meeting job standards in a satisfactory manner.)
- New Hires after April 1, 2008 will be considered in new hire orientation status and do not require an evaluation. New hires in orientation status are not eligible for merit allocations in October.
- All employee Performance Evaluations for employees MUST be completed by the June 30, 2008 deadline. Departments will have until July 15, 2008 to get all the evaluations summarized and submitted to Healthcare Payroll but the evaluations must be performed by or prior to June 30. JCAHO requirement is to record all late evaluation reviews signed after June 30. (All Demographic Data on front page MUST be completed including the employee ID.) There will be a new process established to aid in keeping track of all the scores. A summary sheet of scores will need to be submitted via email to Healthcare payroll. This will be in the form of an Excel worksheet with the following information: 1. Employee ID # (not social); 2. Employee full name (not nick name), last name, first name; 3. Employee score. Summary worksheets must be submitted to the office by the July 15, 2008 date. Submit worksheets to Healthcare Payroll to the following email address: ceburt2@email.uky.edu.
Transfers to and from the Healthcare environment as related to evaluation changes:
- Transfers from the Healthcare to the Campus environment positions:
- Prior to July 1, 2008 -- employees will be required to meet criteria established by campus for evaluations.
- Transfers from Campus to Healthcare positions:
- Prior to July 1, 2008 -- departments will use the transferred Campus department evaluation score. New department will be required to retrieve prior Performance score from the transferred department. Prior department score will be used for evaluation in October. (Read FAQ #2 for full instructions.)
- July 1, 2008 to September 30, 2008 -- employees are not considered for evaluation or merit in October due to already meeting required criteria from Campus as of July.
*** Please remember that all employees on the current employee roster are not in new hire orientation status MUST have a Performance Evaluation. This included ALL on-call and pool staff. Please do not fill out Performance Evaluations on new hires that fall within the dates specified on or after April 1, 2008. ***
Note: The Performance Evaluation form states to send completed forms to Room 21 Scovell Hall. In order to record scores for employees, instead please send copy directly to Healthcare Payroll, H178A (do not send to Scovell Hall). Employee forms will be forwarded to Human Resources at Scovell Hall from Healthcare Payroll once scores are recorded.
